Volunteer Agreement
CMFCAA strives to create a nurturing space where children can grow and heal. In pursuit of this goal, volunteers are expected to keep all information regarding the children and families in our program confidential and to treat everyone with whom we work with respect. If there is a situation in which you feel a staff member should be aware of a problem regarding one of the clients in our program, contact the volunteer manager to determine the best possible solution. Failure to comply with these policies may result in dismissal from volunteer staff and/or civil or criminal prosecution when applicable. By signing this form, I acknowledge that I understand CMFCAA’s volunteer guidelines and expectations of me as a volunteer. I understand that volunteering with CMFCAA is not for a specified term and is at the mutual consent of CMFCAA and myself. I agree to abide by the Policies and Procedures laid out in orientation. By signing this form, I agree to:
• Honor my commitment to the agency
• Cooperate with staff and fellow volunteers
• Be prompt and reliable
• Notify the volunteer manager of any changes to your commitment as a volunteer
• Keep all communications with or concerning the clients in our program confidential
• Maintain a respectful relationship with clients
• Model professional behavior, through both dress and decorum
• Report any issues with CMFCAA staff to our HR representative for help with our grievance policy (interns must report issues with CMFCAA staff to a field supervisor)
• Promote the safety of the children, parents, staff and volunteers by adhering to all rules and guidelines During the time that I serve as a volunteer of Central Missouri Foster Care & Adoption Association (CMFCAA)
Non-Disclosure
I realize that I may gain access to information that is considered to be confidential and/or proprietary. Such information relates to personal information about clients served and/or decisions made with regard to the business of CMFCAA. Since confidential and proprietary information is crucial to the operation of CMFCAA, and because CMFCAA in some instances has the obligation to protect such information, I agree that I will not use, publish or disclose such information during or after my volunteer tenure/time/duty. I will preserve the restricted nature of this information except to the extent that it becomes publicly available, or is otherwise lawfully obtained outside the scope of this agreement from third parties. I hereby certify that I have read, understand and agree to Central Missouri Foster Care & Adoption Association’s policies as described in throughout this agreement.

You may also file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights by sending a letter to 200 Independence Avenue, S.W., Washington, D.C. 20201, calling 1-877-696-6775. We will not retaliate against you for filing a complaint.
